Drug-induced interstitial lung disease (DI-ILD) is an increasingly encountered entity with the rising use of cytotoxic, immunomodulatory, and targeted therapies. Radiologists are pivotal in early identification and differentiation of DI-ILD from other causes of lung infiltrates. This presentation reviews key high-resolution CT (HRCT) patterns—including NSIP, organizing pneumonia, diffuse alveolar damage, and hypersensitivity pneumonitis—commonly associated with drug toxicity. The session will also discuss imaging–clinical correlation, temporal patterns post drug exposure, and practical tips for narrowing differential diagnoses. Timely recognition of radiological findings can facilitate drug cessation and intervention, ultimately improving patient outcomes.
