Bronchiectasis is a chronic airway disease characterized by irreversible dilatation of the bronchi, often resulting from recurrent infections and immune dysfunction. In adults, primary immunodeficiency (PID) is an under-recognized but significant etiological factor contributing to the development and progression of bronchiectasis. This presentation will explore the pathophysiological links between PID and bronchiectasis, emphasizing the importance of early recognition and comprehensive diagnostic evaluation. We will review current diagnostic algorithms, including immunological assessments and imaging modalities, to identify underlying immune deficiencies. Additionally, evidence-based management strategies tailored to patients with PID-related bronchiectasis will be discussed, highlighting the roles of immunoglobulin replacement therapy, airway clearance techniques, and targeted antimicrobial treatments. Recognizing the interplay between immune deficiency and bronchiectasis is crucial for optimizing patient outcomes, reducing exacerbations, and improving quality of life. This session aims to enhance awareness among clinicians, promote multidisciplinary collaboration, and foster personalized approaches to care in this complex patient population.
