Paediatric lung transplantation is now in its 5th decade and has become an established therapy for end stage lung disease in children in many countries. This talk will cover how modern services operate, from donor offer through to transplant surgery, peri-operative care, and follow up, including consideration of the ethical choices in candidate selection and donor-recipient matching, machine perfusion approaches to increase organ availability (with ex-vivo lung perfusion), infant transplant, and discuss long term outcomes as well as the challenges in post transplant care.
