Introduction
Asthma affects approximately 6.3% of Malaysian adults. Uncontrolled asthma which leads to increased clinic visits and admissions is observed in 8.9-13.0% in children and 21% of the adolescents.
Objectives
A budget impact analysis (BIA) of transitioning from a multiple-inhaler triple therapy to single inhaler among the uncontrolled the adult asthma patients was carried out in the Kedah State Health Department (KSHD), Malaysia.
Methodology
A budget impact model was built, where uncontrolled asthma patients (step 4 and 5 of the GINA guidelines 2024) on multiple inhalers [(Corticosteroids (ICS) and long-acting beta-agonists (LABA) plus long-acting muscarinic antagonists (LAMA)] were transitioned to single inhalers containing similar ingredients in the hospitals in KSHD. The BIA was conducted from the perspective of the KSHD and estimated the annual financial impact over a period of 5 years.
Results
A total of 168 patients meeting step 4 and 5 of the GINA guidelines were identified for the base year based on the primary data collection form the facilities in Kedah. The budget impact model included the clinic and emergency department visits, hospitalization drug and disease related costs. The budget impact of transitioning to the single inhalers estimated to be a savings of MYR 243,349 over a period of 5 years.
Conclusions
The BIA estimated a reduction in admissions and substantial savings in the overall treatment costs. The number of patients who would have access to the single inhalers over the next 5 years were increased, based on these estimates.
